摘 要 目的: 以关节镜检查结果 为标准 , 分 析评 价 3. 0T 磁 共振 3D F S SPGR 序 列对 膝关节 软骨 损伤 的诊断 价值。
方法: 对 将要进 行关 节镜 检查的 50 个膝 关节进 行磁 共振多 序列、多 方位 成像, 包括 矢状 位 FSE T 2 WI 、FSE T 1 WI、F S
Abstract Objective: T o investig ate the diag nostic v alue o f differ ent M RI sequences ( FSE T 2 W, F SE T 1 WI, F SE F S T 2 WI and 3D F S SP GR ) in detectio n o f the articula r cartilag e injury of knee joints compared w ith the ar thro sco pic find ing s. Methods: Sag ittal FSE T 2 W I, sagittal F SE T 1 WI, sag itta l FS F SE T 2W I, sag ittal 3D F S SPGR , cor onal F S FSE T 2 WI, ax i F S FSE T 2 WI, and a rthro sco py w ere perfor med in 50 patients w ith art icular cartilag e injur y in knee joints. Sag itt al 3D FS SPG R imag es w ere r eco nstr ucted w ith M PR. In addition, 20 nor mal knee jo ints of healthy vo lunt eer s un derw ent M RI. All the M R images o f cart ilag e injur y of t he medial and later al femora l condyles, medial and later al tibial plateaus, femoral tr ochlea and patella wer e compar ed with r esults o f a rthro sco pic ex amination. Results: O n F S FSE T 2 WI, the articular cartilag es o f the no rmal knees presented as smo oth curv e like hy per intensity. T he sig nal of ar ticular ca rtilages o f media l and lateral femo ral co ndyles, medial and lateral t ibial plateaus consisted of clear thr ee layer ed st ructur e sho wing high low high intensity fr om superficial to deep lay er. M eanw hile, the articular ca rtilages of knees sho wed as un clear tw o layered moderately intensity o n FSE T 2 WI and F SE T 1 WI. Evident band like hy per intensity with three lay ered st ruct ur e of hig h low hig h intensity from superficial to deep layer wer e sho wed on 3D F S SP GR; T he articula r cartila g es of Gr ade injur y presented as low or hig h intensity on F S F SE T 2 WI, lo w intensity o n 3D F S SP GR wit ho ut lamina t ing . T he a rticular ca rtilage o f G rade ~ injur y presented as moderately intensity on FSE T 1 W I, hig h intensity on FSE T 2 W I and F S F SE T 2 WI, low intensity o n 3D FS SPG R; Co mpar ed with ar throscopic result, the sensitiv ity , spe cif icity and Kappa wer e 91. 4% , 95. 9% and 0. 808( > 0. 75) respectively w ith 3D F S SP GR sequence; 88. 9% , 96. 8%
显示膝关节软骨损伤优于 F SE T 2 W I、F SE T 1 WI、FS FSE T 2 W I 序列, 为诊断膝关节 软骨损伤的最佳序列。
