随着社会的不断发展与进步，现代化建设、高速公路里程的 不断延伸及交通工具的不断现代化，导致的严重复合伤患者不断 增多，以多发性肋骨骨折为主，兼以其他严重复合伤的急诊抢救 尤其显得更为重要。多发性肋骨骨折及其复合伤成为胸部创伤死 亡的高危因素，且传统非手术治疗的缺点使得医学工作者意识到 探索新的治疗方法的必要性。本院于 2008 年 4 月至 2011 年 4 月 对收治的多发性肋骨骨折 45 例患者应用可吸收肋骨钉联合 镍钛 合金环抱器进行内固定治疗，取得了较好效果，现分析如下。

可吸收肋骨钉联合镍钛合金环抱器在多发性肋骨骨折 内固定术中 45 例应用分析

【摘 要】 目的 探讨可吸收肋骨钉联合镍钛合金环抱器内固定治疗多发性肋骨骨折的适应证、手术方法、疗效及 可行性。 方法 回顾性分析 该院 2008 年 4 月 至 2011 年 4 月 收 治的 45 例 多 发性 肋 骨 骨折 患 者 ，用可 吸 收 肋骨 钉 联 合镍 钛 合 金环 抱器在全麻下行肋骨内固定的手术方法及治疗情况。 结果 45 例患者中 41 例 胸 廓 全 部 恢 复 正 常 ，4 例 由 于 病 情较重，只选择胸壁软化区关键肋骨固定，术后胸廓稍有畸形，但均无严重术后并发症 。术后 3 个月骨折基本愈合，3~6 个 月恢复正常活动。 结论 可吸收肋骨钉联合镍钛合金环抱器治疗多发性肋骨骨折适应证广，手术操作简单，创伤小，恢复 呼吸道功能效果显著，有较好的疗效，值得推广应用。

Application analysis on absorbable rib pin combined with nickel titanium alloy embracing fixator in 45 cases of multiple ribs fracture internal fixation Chen Jianzhong，Cheng Zhengjun（First Department of Surgery，Second People's Hospital of Jiulongpo District，Chongqing 400052，China）
【Abstract】 Objective To investigate of the indications，operative method，effect and feasibility of the absorbable rib pin combined with nickel titanium alloy embracing fixator in treating multiple ribs fracture. Methods 45 patients with multiple ribs fracture in this hospital from April 2008 to April 2011were retrospectively analyzed on the operative method and treatment status by the absorbable rib pin combined with titanium nickel alloy embracing fixator. Results Among 45 cases，41 cases with thoracic deformities recovered to normal. 4 cases only selected the key rib fixation in chest softened area due to severe condition and had slight chest deformity after operation without serious postoperative complications. Fractures were basically healed in postoperative 3 months and the normal activities were recovered in postoperative 3-6 months. Conclusion Absorbable rib pin combined with titanium nickel alloy embracing fixator for treating multiple ribs fracture has the wide indications ，simple operation， small injury， obvious recovery of respiratory function and better curative effect，and deserves to be popularized.
1 资料与方法
1.1 一 般 资 料 45 例 患 者 中 男 31 例 ，女 14 例 ，年 龄 16~75 岁 ， 中 位 42.5 岁 ；车 祸 38 例 ，高 处 坠 落 伤 7 例 ，均 为 多 发 伤 。术 前 常 规经胸部 X 射线片、CT 三维重建确诊：9 例为双侧多发性肋骨骨 折，36 例为单 侧 多 发 性 骨 折 ，骨 折 最 多 16 处 ，最 少 3 处 ；并 发 创 伤性血胸 30 例，创伤性血气胸 15 例，并发肺挫伤 36 例，肾挫伤 2 例，颅脑损伤 1 例，肝、脾破裂各 1 例，并发其他部位骨折 19 例。 1.2 治疗方法 1.2.1 术 前 处 理 入 院 后 立 即 给 予 吸 氧 、心 电 监 护 、抗 感 染 止 血
