方法:21只新鲜尸体手标本分别血管灌注染料解剖及动脉铸型标本对照观察,7个拇指背尺侧皮肤组织学切片检查。
结果:(1)拇指背尺侧动脉在甲襞近侧与甲襞动脉弓相吻合,又在拇指近节指骨颈水平通过穿支与拇指掌侧动脉吻合,并与拇指背尺侧皮神经呈节段伴行;拇指背尺侧动脉又通过“微小血管吻合”连接第1掌骨背处尺侧皮神经旁血管。
(2)拇指背尺侧动脉无紧密伴行的静脉。
结论:Bertelli的“拇指背尺侧(带)皮神经岛状皮瓣”和Brunelli的“拇指背尺侧皮瓣”均以拇指背尺侧动脉逆向供血,静脉回流赖以拇指背浅静脉及指深静脉逆向返流,但前者是后者的拓展。

两者关系如何?未见报道。
作者通过对21只新鲜尸体手标本的解剖及7个组织学切片检查加以探讨。
1 材料和方法成人新鲜尸体手标本25只,分别:(1)8只墨汁胶浆灌注后,解剖观察拇指背尺侧动脉及相关吻合血管来源、行径、分布,拇指背尺侧皮神经的伴行血管和营养血管情况。
(2)10只动脉灌注ABS(丙烯腈丁二烯塑料)动脉铸型标本和上述结果进行对照观察。
(3)3只静脉灌注染料解剖拇指及第1掌骨背静脉分布情况。
(4)上述3只动脉墨汁灌注标本和4只新鲜尸手标本取拇指背尺侧皮肤组织作HE染色组织学切片检查。

拇主要动脉在穿出第1骨间肌处发出一支拇指背尺侧动脉,外径0.3～0.9mm。
本组8只墨汁灌注解剖标本仅1只例外,其由桡动脉腕背段主干经第1骨间背侧肌浅面直接发出(图1,2)。
拇指背尺侧动脉较为恒定,位于拇指背面的尺侧,由第1掌骨颈下行至近侧甲襞动脉弓。
其在近侧甲襞近端0.5～1.0cm处,通过近侧甲襞动脉弓与拇指掌侧动脉的分支吻合;又在近节指骨颈水平通过交通支与拇指尺掌侧动脉、桡掌侧动脉吻合(图3)。
2.2 动脉与拇指背尺侧皮神经伴行情况拇指背尺侧动脉经过第1掌指关节后与拇指背尺侧皮神经紧密伴行至甲根(图2),由桡动脉腕背段的小分支形成的拇指背尺侧皮神经旁血管丛与拇指背尺侧动脉之间通过微小血管网互相吻合。
2.3 拇指背侧的静脉拇指背侧浅静脉起始于甲沟和甲襞处的小静脉,在甲根至指间关节之间合成4～8条,这些小静脉行至掌指关节水平逐渐合成2～3支直径为2～3mm的静脉,并在近节指骨背近1/2内相互交通;静脉过掌指关・44・CHIN ESE JOURNAL OF CL IN ICAL ANA TOM Y VOL.21 NO.1 2003节水平后,在拇短伸腱两侧各有一条同行静脉,两者在第1掌骨背处交通构成2～3个弓;在指间关节水平和掌指关节水平浅深静脉有侧支交通。
拇指背尺侧动脉无紧密伴行的静脉(图4)。

(2)皮神经干内血管网,由神经营养动脉上行支吻合构成,为次要供血。
本解剖观察表明:“拇指背尺侧(带)皮神经岛状皮瓣”动脉血供主要来源于拇指掌侧动脉的分支,其与拇指背尺侧动脉吻合逆行供血皮瓣,又通过“微小动脉吻合”连接了第1掌骨背处拇指背尺侧皮神经旁血管丛,从而拓展供血距离即扩展皮瓣的蒂长和/或皮瓣长度。
