在本条款中，本公司指合众人寿保险股份有限公司Inthisprovision,thecompanyreferstounionLifeInsuranc

1.投保人与本公司的合同

1.1投保范围
1.1scopeofInsurance
凡在本附加保险合同（以下简称“本附加合同”）订立时对被保险人具有保险利益或经被保险人书面同意的机关、企业、事业单位和社会团体均可作为投保人。

凡符合本公司承保条件的人均叫作为被保险人。
Allthosecomplyingwiththeinsuranceconditionscanbecon sideredastheinsured.
1.2合同构成
1.2contractFormation
本附加保险合同需由主保险合同(以下简称“主合同”）投保人提出申请,经本公司同意而订立。
ThisAdditionalcontractshallbesignedoncebeingapplied bytheinsuredsetinthemasterInsurancecontract(hereina fterreferredtoas
“mastercontract”)andbeingagreedbythecompany.
主合同所附条款、投保书、合法有效的声明、批注、附贴批单、其他书面协议，凡与本附加合同相关者，都是本附加合同的构成部分。若主合冋与本附加合同的条款互有冲突，则以本附加合同的条款为准。
ThisAdditionalcontractincludesallprovisionssetinthe mastercontract,proposalform,legalandvaliddeclaratio n,notes,approvalordersattachedtothisAdditionalcontr act,otherwrittenagreementsandpartiesrelatedtothisAd ditionalcontract.IfthereisaconflictbetweenthisAddit ionalcontractandmastercontract,Additionalcontractsh allbeprevailed.
1.3投保人解除合同的手续及风险
1.3FormalitiesandRisksofTerminatingthecontractbypol

如投保人申请解除本附加合同，请填写解除合同申请书并向公司提供如下资料：
IfthepolicyholderappliestoterminatethisAdditionalco ntract,itisnecessarytofilltheapplicationformforcont ractterminationandsubmitdocumentslistedherebelow:
(1)保险合同:
(1)Insurancecontract
(2)保险费收据。
(2)Insurancefeereceipts
自本公司收到解除合同申请书时起，本附加合同终止，本公司自收到解除合同申请书之日起30日内向投保人退还保险单的现金价值（见释义6.1）.
ThisAdditionalcontractwillbeterminatedfromthedaywhe nthecompanyreceivestheapplicationofterminatingcontr act.Thenthecompanyshallrefundthecashvalue(refertoex planation6.1)oftheinsurancepolicytothepolicyholderw ithin30daysuponreceivingtheapplication.
2.本公司提供的保障

2.1保险金额本附加合同被保险人的保险金额由投保人与本公司约定并在保险单中载明。

同)lbeagreedbythepolicyholderandthecompany,whichsha llbeclearlydescribedintheinsurancepolicy.
2.2保险期间本附加合同的保险期间同主合同。
2.2Insuranceperiod.TheinsuranceperiodofthisAddition alcontractisthesameasthatofthemastercontract.
2.3保险责任在本附加合同保险期间内，本公司承担下列保险贵任:
2.3InsuredLiability.Duringtheinsuranceperiod,thefol lowingliabilitiesaretobeundertakenbythecompany: 若在理赔时，被保险人己从社会基本医疗保险、公费医疗或其他途径（包括：工作单位以及包含本公司在内的任何商业保险机构费用补偿型产品等）获得补偿，本公司将在扣除前述补偿后，按剩余的部分给付意外伤害医疗保险金。
