禁止使用物質的不使用証明書
本公司對現在或將來提供給宏塑工業股份有限公司及其集團所屬各公司（以下簡稱宏塑公司）的原材料、配件、添加劑、包裝材料作出以下聲明：
1.本公司所提供宏塑公司之相關產品，皆符合歐盟RoHS禁止在電子電器設備中使用鉛、鎘、汞、
六價鉻（hexavalent chromium）等四種重金屬以及PBB和PBDE等兩種溴化物阻燃劑（brominated flame retardant）。

2.上述提到之有毒物質含量須符合歐盟2004 年9 月列出濃度上限建議如下：
．相同材質所含的鉛、汞、六價鉻、PBB 和PBDE，最高濃度不得超過重量的0.1 %(1000PPM) ．相同材質物料所含的鎘，最高濃度不得超過重量的0.01 %(100PPM)
．包裝材質物料所含的鉛、鎘、汞、及六價鉻的最高濃度水準之總和不得超過0.01 %(100PPM)
3.宏塑公司之客戶如有其他禁限用物質要求者，需依其管制。

4.如果宏塑公司要求本公司產品之重金屬測定數據和成份表，本公司須於出貨前無條件提供國際
認可實驗室的第三方驗證報告(限定SGS,ITS,ETC,TUV)以証明產品重金屬含量在允許濃度以內。

5.本公司在生產提供宏塑公司的產品中，其原物料及製程(含外包)、包材所使用添加劑中或表面加
工處理之有毒物質含量須符合歐盟要求，若要變更使用材料或改變生產條件、製造地、外包商及其他可能影響產品的成份時，須再取得國際認可實驗室的第三方驗證報告(限定SGS,ITS,ETC,TUV)確定沒有違反上述所列的管制物質，並在取得宏塑公司核准後，再予以生產出貨。

6.本公司注意對宏塑公司依據歐盟RoHS所列有害物質進行管制，並保証2005/6/30 期限時停止提
供含此類物質的產品。

7.此聲明生效後，若本公司提供給宏塑公司之產品不符合本聲明，本公司愿承擔法律及經濟責任。

3)Cadmium (Cd) ≦0.01%(100ppm)
4)Hexavalent chromium (Cr (VI)) ≦0.1%(1000ppm)
5)Polybrominated biphenyls (PBB) ≦0.1%(1000ppm)
6)Polybrominated diphenyl ethers (PBDE) ≦0.1%(1000ppm)
7)”Less than 100ppm” is determined as the allowable total-concentration of four heavy metals
(mercury, cadmium, hexavalent chromium, and lead contained in each part.)
The above products have been compliant since product inception. No changes have been made in process or materials. All products sold under the above P/Ns previously, currently and in future are/will be fully compliant with EU Directive 2002/95/EC & 94/62/EC (effective date from 2005/6/30).
If we are/will not be fully compliant with EU Directive 2002/95/EC and Foreshot restrict using the matrix of hazardous substances, we bear the sole legal and the economical ramifications of our non-compliance and so forth hold Foreshot Industrial Corporation and its affiliates harmless and not liable.
For the further reference, please advise the of laws and regulations the European Union Directive 2002-95 and Foreshot restrict using the matrix of hazardous substances, as provided below and attached.
