企业职业健康体检协议
体 检 协 议
甲方:
乙方:
根据《中华人民共和国职业病防治法》及相关法律的要求和规定，为保证职工
身体健康，有效的防治职业病的发生，甲方委托乙方对贵厂接触职业性有害因素的
职工进行职业性健康体检，为保证此项工作的顺利进行，经双方共同协商，达成以
下协议条款。
一、检查项目:见附表
二、检查时间:2015年7月下旬(另行商定)。
三、体检费用:按实际检查人数结算。
四、本次体检采用上门服务形式，由甲方负责提供体检场所、检查用床、
桌椅及合适的电源，对体检职工进行组织协调工作，保证体检的顺利
进行。
五、乙方检查时要认真负责，检查结果必须明确，有疑问的须下建议结论，
提出是否复查和治疗的建议。
六、体检结束后，甲方收到体检报告后应在一个月内一次性付清全部费用。
七、体检注意事项:参加体检职工当天应空腹，体检前三日内应禁酒。 八、检查结
果由乙方通知甲方，不得向外泄露。
