尿NGAL和Kim—1对2型糖尿病肾病早期诊断的意义
目的探讨2型糖尿病(T2DM)患者尿NGAL和Kim-1的改变及其对糖尿病肾病(DN)早期诊断的临床意义。
方法选择2型糖尿病(T2DM)患者80例,根据尿清蛋白排泄率(UAER)分为3组,正常清蛋白尿(NA)组20例、微量清蛋白尿(MA)组35例、临床肾病(CN)组25例;同期选取非肾病患者20例作为对照组。
应用ELISA法检测患者尿NGAL(uNGAL)水平,应用免疫速率散射比浊法检测血清Cystatin C水平,分析尿NGAL及血清Cystatin C水平与肾小球滤过率(GFR)之间的相关关系。
结果①MA组、CN组患者尿NGAL 和血清Cystatin C水平均较对照组升高,差异有统计学意义(P<0.01),其中CN 组患者尿NGAL和血清Cysta tin C水平最高分别为(95.56±32.68)ug/L和(4.73±1.62)mg/L。
②Pearson相关分析:糖尿病患者尿NGAL和血清Cystatin C 水平与GFR均呈负相关(r值分别为-0.312和-0.412,P<0.05),且尿NGAL与GFR的相关性更密切。
结论糖尿病患者尿NGA与GFR有较好的相关性,尿NGAL和血清Cystatin C是诊断早期DN敏感而准确的指标,有望用于临床监测。
Abstract:Objective Explore the type 2diabetes mellitus(T2DM)patients serum NGAL,Cystatin C change and to the diabetes nephrosis(DN)earlydiagnosis of clinical significance.Methods Selection type 2diabetes mellitus(T2DM)in 80cases,According to the urinary albumin excretion rate(UAER)intothree groups,Is temperamental predominance proteinuria(making)group of 20cases,trace qing proteinuria(MA)group 35examples,clinical disease(CN)group in 25;Select the same period the kidney disease patients 20cases as control group.Application ELISA detection of patients with urinary NGAL(uNGAL)level Application immune rate than scattering turbidity method to detect serum Cystatin C level,analysis and the Cystatin urine NGAL C level and estimated glomerular(GFR)the relationship between.Results ①MA group,CN groups of patients NGAL urine and serum Cystatin C level than control group,the difference was statistically significant increased(P<0.01),including CN groups of patients NGAL urine and serum Cysta tin C level is the highest for respectively 95.56+/-32.68ug/Land(4.73±1.62mg/L.(2)Pearson correlation analysis:patients with diabetes NGAL urine and serum Cystatin C level and do all negative correlation(P<0.05),and NGAL and the correlation of urine do more closely.Conclusion Diabetic patients with urinary NGAL do have good correlation,urine NGAL and serum Cystatin C is sensitive and accurate diagnosis of early DN index,is expected to used for clinical monitoring.

中性粒细胞明胶酶相关脂质运载蛋白(NGAL),近年有研究认为该蛋白有可能参与糖尿病的发生发展。
研究表明其不仅参与多种肾脏病变过程中表现突出,还参与肾脏形态的形成及修复。
胱抑素C(Cystatin C)是一个比较接近理想的内源性指标。
本研究检测DN患者尿NGAL和血清Cystatin C水平,并评价它们对DN早期诊断的意义。
1资料与方法
1.1一般资料按WHO的糖尿病诊断和分型标准,收入我院2011年1月~4月2型糖尿病(T2DM)患者80例。
按照尿清蛋白排泄率(UAER)分为正常清蛋白尿(NA)组20例,UAER200Lg/min,男11例,女14例,平均年龄(61.3±6.4)岁。
对照组选取20例无其它的疾病,男12例,女8例,平均年龄(56.5±5.8)岁。
