动火作业许可证
Notes/备注：
1. "Hot Work Permit" must be borrowed from FM safety member and put on site after application approval;
动火申请得到批准后，在厂区FM安全组领取“临时动火证”，放在动火现场；
2. If hot work content change, re-application is a must;
焊接对象变动时，需重新申请及领取“临时动火证”；
3. Application must be issued at least one day before work start. Emergent hot work also must get approval in advance;
一般临时动火至少提前一个工作日审批，紧急临时动火，需获得厂区FM安全组批准。

4. "Hot Work Permit" must attach valid welding certificate. The permit period limited within three day.
动火申请需附上焊工证原件，其有效期限最长为三天。

5. The form apply to internal operate
此申请表同样适用于公司内部动火作业。
