• Writing an effective questionnaire is not a task for novices. At the very least, it requires an understanding of four basic issues: First, you have to consider the differences between a questionnaire that respondents fill out themselves and one that a professional interviewer administers. Second, you have to know what questions should be asked early on in the questionnaire, in the middle, and toward the end. Third, you have to understand how to phrase questions. Fourth, you have to be sensitive to questionnaire length or they might lose interest in answering the questions. Include a cover letter or introduction with the survey. Let the recipient know what you’re trying to accomplish with the survey and how valued his response is.

• Questionnaires have advantages over some other types of surveys in that they are cheap, do not require as much effort from the questioner as verbal or telephone surveys, and often have standardized answers that make it simple to compile data. However, such standardized answers may frustrate users. Questionnaires are also sharply limited by the fact that respondents must be able to read the questions and respond to them. Thus, for some demographic groups conducting a survey by questionnaire may not be practical. As a type of survey, questionnaires also have many of the same problems relating to question construction and wording that exist in other types of opinion polls.
