常规免疫接种率报告和监测系统评价Evaluation on Surveillance Data for Routine Immunization Coverage Reports and Monitoring Systems in Gaomi in 2011 Gaomi City CDC, Li X, Li ZZ, Zhang YD, Deng ZH. Abstract: [Objective] To evaluate the status of routine immunization coverage reporting in Gaomi city in 2011. And find the problems and put forward rectification opinions, in order to promote the future work. [Methods] With the methods of the estimated routine coverage rates; the difference between reported coverage rates and estimated ones (D value); the ratio between reported coverage rates and estimated ones (R value), etc. The routine immunization coverage rate monitoring data results of nine kinds of vaccine were analyzed and evaluated. Those vaccines of Immunization Program Information Management System (IPIMS) are reported by 15 vaccination clinics. [Results] Complete rate of the city reported is 100%. The timeliness rate is 98.3%. The accuracy of the data rate is 95%. Basic immunization reported vaccination rate above 95%. The vaccination rates are about more than 100%. Those data is too high. And all the D values are all too much high and statistics show that the data are incredible. But the R1、R2、R6、R7、R8 values are credible. R4、R5、R9、R10 are doubtful and R3、R11、R12 are incredible. [Conclusions] The full rate, time rate, accuracy rate of monitoring report and the report of immunization coverage rate are a little higher. This indicates that the vaccination rate monitoring and reporting system has aroused the attention of the basic units. The quality of statements member are enhanced and the quality of report is improved. But the credibility of the D value and R value are low. [方法]用估计接种率、差值（D值）和比值(R值)评价方法，对全市15处接种门诊上报的，中国免疫规划监测信息管理系统9种疫苗的，常规免疫接种率监测数据结果进行分析评价。
[结果]全市报告完整率为100%，及时率为98.3%，数据准确率为95%；基础免疫报告接种率95%以上；估算接种率都超过100%，数据偏高；D值全部偏高，统计均显示不可信；R1、R2、R6、R7、R8显示可信，R4、R5、R9、R10显示可疑，R3、R11、R12显示不可信。
[结论] 监测报告完整率、及时率、准确率，报告接种率较高，表明接种率监测报告系统已经引起基层单位的重视，报表员素质提升，报告质量提高；估计接种率、D值和R 值可信率较低，说明基层数据有漏报，缺报、掌握不全现象。

我们对2011年卡介苗（BCG）、乙肝 (HepB) 、脊灰（PV）、百白破三联（DPT）、含麻疹类(MCV)、白破二联（DT）、流脑A（MENA）和乙脑（JE）、甲肝 (HepA)9种疫苗的常规免疫接种率监测数据进行分析评价，现将结果报告如下：1 资料来源：2011年1-12月全市15处接种门诊上报的免疫接种率数据库导出的数据2 分析评价方法：2.1 常规基础、加强免疫报告情况全市15处接种门诊有12处每月运转3次，市直、市立和开发区接种门诊每日接种。
数据每月上报1次；监测信息管理系统应收集1-12月份接种率报告180份，无缺漏报算完整；每月最后一次运转完毕3日内上报数据为及时，以邮箱收到时间为准；每月次数据无差错为准确；每月次上报数据无缺项算数据完整。
2.2 估算接种率、接种率D值、R值评价2.2.1 接种率评价方法依据：根据《全国常规免疫接种率监测方案》进行估计接种率、差值（D值）和比值(R值)评价。
【1】2.2.2 接种率评价方法：2.2.2.1 估算接种率：以市统计局公布的2010年的出生人数为估计应接种人数，以监测系统报告的实际接种人数为实种人数计算估计接种率；2.2.2.2 D值：D值=┃估计接种率—报告接种率┃；当D≤0.05时为可信，0.05＜D≤0.15为可疑，D＞0.15为不可信；2.2.2.3 R值：R值≈3MV(BCG)∕OPV（DPT、HBV）；R1=3MCV/（PV1+PV2+PV3）； R2=3MCV/（DPT1+DPT2+DPT3）；R3=3BCG/（PV1+PV2+PV3）；R4=3BCG/（DPT1+DPT2+DPT3）；R5=3MCV/（HepB 1+ HepB 2+ HepB 3）；R6=3BCG/（HepB 1+ HepB 2+ HepB 3）；R7=2MCV/（MENA1+MENA2）； R8=MCV/JE；R9=2 BCG /（MENA1+MENA2）；R10= BCG /JE；R11=2 MCV /（HepA 1+ HepA 2）；R12=2 BCG /（HepA 1+ HepA 2）.当0.95≤R≤1.05时为可信，0.90≤R＜0.95或1.05＜R≤1.15时为可疑，R＜0.90或R＞1.15为不可信。
2.3 报告接种率：某疫苗报告接种率＝某疫苗报告实种人数/该疫苗报告应种人数×100％。
3 结果与分析：3.1 接种率报告完整率、及时率、准确率情况及分析：3.1.1全市看，15处接种门诊报告完整率为100%；及时率为98.3%，除姜庄83.3%、呼家庄91.7%外，其余乡镇为100%；数据准确率为95%，柏城、康庄、井沟91.7%，大牟家、姚哥庄、呼家庄83.3%，其余乡镇100%；数据完整率为99.4%，除大牟家91.7%，其余乡镇100%。
详见表13.1.2 结果表明，接种率监测报告系统已经引起基层单位的重视，纳入常规工作日程。
报表员素质提升，报告质量提高。
但仍有少数乡镇出现迟报、漏报、错报现象。
3.2 常规基础免疫接种情况与分析3.2.1 基础免疫报告接种率3.2.1.1全市合计，基础免疫报告接种率分别为HepB1 100%（8898/8898），及时率99%（8796/8898），HepB299.6%（9080/9090），HepB398.9%（9316/9330）; BCG 100%（8882/8882）;PV199.1%（9618/9633），PV299.2%（10245/10260）, PV3 99.4%（10682/10701）; DPT1 99.1%（9532/9547）， DPT2 99.2%（9777/9798），DPT3 99.1%（9840/9864）;MCV1 99.8%(9708/9725);MENA1 99.1％（9826/9844），MENA298.5％（9549/9565）;JE1 99％（9863/9876）;HepA199.8%（8203/8213）.3.2.1.2全市基础免疫报告接种率95%以上；乡镇除呼家庄HepB393.9％外，其他都在95％以上。
说明以本系统自己掌握的儿童数为基数，接种率较高，基本无漏种漏报现象。
详见表23.2.2 基础免疫估算接种率3.2.2.1 全市估算接种率分别为HepB1119％、HepB2120.3％、HepB3122.5％、BCG 118.9％、PV1128.1％、PV2 135.1％、PV3140％、DPT1124.1％、DPT2126.5％、DPT3128.6％、MCV1131％、MENA1128.8％、MENA2125.7％、JE1130.5％、HepA1 108.2%。
3.2.2.2 全市估算率都超过100%数据偏高。
市直PV1、DPT1；拒城河PV1、DPT1、HepA1，柴沟HepA1、JE1在90％—100%之间；柏城HepB1、HepB2、BCG，市直HepB1、HepB2、BCG，市立MV1、HepA1，河崖HepA1，大牟家HepB1、MENA1、MENA2，开发区HepB1，呼家庄HepB1，柴沟HepB1、BCG、MENA1、MENA2估算接种率均达不到90％。
