温州医学院硕士学位论文腰丛一坐骨神经阻滞时利多卡因对罗哌卡因药代动力学的影响摘要
目的建立同时检测人血浆利多卡因和罗哌卡因的高效液相色谱分析方法,观察下肢手术患者腰从一坐骨神经阻滞时利多卡因对罗哌卡因药代动力学的影响。方法l、高效液相色谱仪为Agilent1100系列:血浆碱化后经乙酸乙酯萃取浓缩,以ZORBAXEclipseXDB—Cls(4.6x150mm,59m,Agilent,USA)
为色谱柱:以乙腈.O.02mol·L—KH2P04(16:84,V/V)为流动相,流速为1.0ml·min~;检测波长为210nm;内标为布比卡因。2、择期行膝及膝以下手术患者20例,ASAl~ll级,随机分为两组,分别为:单用罗哌卡因组(R组),利多卡因和罗哌卡因联用组(LR组),每组lO例。神经刺激器引导下定位坐骨神经、腰丛,R组先注射生理盐水10ml再追加0.75%罗哌卡因10mi行坐骨神经阻滞;LR组采用先注射2%利多卡因10ml再追加0.75%罗哌卡因10ml行坐骨神经阻滞;两组均采用0.5%罗哌卡因30ml行腰丛阻滞。从局部麻醉给药开始前、给药后5、10、15、20、30、45、60、120、180、360min采集桡动脉血2ml。反相高效液相色谱法分析各点血药浓度,用药物与统计软件DrugAndStatisticsversion2.0(DAS)分析药代动力学参数,绘制药物浓度一时问曲线,并采用双向单侧t检验、Wilcoxon非参数检验和成组f检验对两组药代动力学参数进行比较。结果l、利多卡因和罗哌卡因浓度在0.05"--10mg·Ld范围内线性关系良好
(r=0.999);最低检测限为0.05mg·L~:两种药物高中低3个浓度的平均相对回收率分别为(99.84-2.9)%和(100.3+3.O)%;平均绝对回收率分别为(68.钍4.6)%和(70.94-4.9)%;日内精密度变异系数分别为
(0.864-0.42)%和(1.36士0.77)%,日间精密度变异系数分别为(2.074-1.46)%和(1.434-0.23)%。
2、R组、LR组罗哌卡因血药浓度峰值(C一)分别为(2.6±O.9)mg/L和(3.1±1.1)mg/L,达峰时间(Tn瞰)分别为(20±15)min和(22±12)min:血药浓度曲线下面积AUCO-t)分别为(422±81)mg·L-krain"1和(588±228)mg·L.1·rain~,AtJG嘶)分别为(644±293)mg·L.。·min"1和(993±638)mg·Ld·rain~;半衰期(tin)分别为(223±169)rain和(264±154)rain;清除率(CVF)分尉为(0.009±0.004)L·min。·kg-1和温州医学院硕上学位论文(0.006+O.004)L·min·1.kg一。3、联用组罗哌卡囚的AUC‘口t)、AUCt0.oo)和C嘲。均显著大于单用组(高侧P<0.05,低侧P>0.05),两组罗哌卡冈Tn似、tl,2和CIdF比较无统计学意义(胗O.05)。
